HeartFit Program Information

HeartFit Introduction

HeartFit for Duty LLC (HeartFit) began as a unique and innovative program specializing in the prevention of heart attacks and strokes in the first responder population by focusing on early detection and education of at risk individuals. Recent studies have shown that first responders have up to a 300% increased incidence of cardiac disease when compared with the normal population. Additionally, research reports as many as 51% of death in the public safety are attributed to cardiovascular disease. We believe a new personalized approach in early detection, primary and secondary prevention is warranted in the high risk population of first responders.

HeartFit will focus on the following six major areas and will include education and resource information to identify and improve modifiable risk factors.

  • METABOLIC DISEASE

  • HYPERTENSION/CARDIO-VASCULAR Risk Factors

  • OBESITY

  • STRESS ASSESSMENT

  • ADVANCED LIPIDS (cholesterol screening and management)

  • DIET/EXERCISE EVALUATION AND PLANNING

    Our program combines education and heart health in a setting that focuses on the individuals to promote healthy lifestyles and long term habits. We specialize in all areas of public safety and the risk factors associated with their roles. We provide individualized wellness programs, provide support and help ensure the success of our patients.

HeartFit Program Information

  • Provide each of you with the education and tools that are needed to create a personalized program designed to improve and/or maintain your health and fitness levels to meet the physical demands of law enforcement.

  • Continue to support your agency, its members and their families in health and wellness. Who we are:

    Medical Director:- Rakesh "Rocky" Patel, M.D.

    Rocky is the medical director for HeartFit For Duty. Rocky is a board certified family practitioner with advanced knowledge of metabolic and cardiac risk modification as well as diabetes reversal to our first responder community. Rocky utilizes his occupational medicine background, his extensive lipid training and education to identify, prevent and reverse disease in our first responders.

    Cardiology Nurse Practitioner:- Megan McCarthy CVNP-C

    Megan is the ONLY board certified Cardiovascular Nurse Practitioner in the state of Arizona and currently is serving as the American College of Cardiology Arizona State Liaison for Nurse Practitioners and Physician Assistants. She serves as a Sub-Investigator on the HeartMatters Challenge Research study in AZ and MA first responders. Megan specializes in the education and treatment of the first responder community by sharing her knowledge of cardiac disease and associated risk factors to help change statistics and keep our first responders healthy, safe and working.

    Registered Nurse – Educator – Kepra Jack RN

    Kepra is a registered nurse who has specialized in emergency medicine for the last 13 years while working closely with the first responder community. Kepra brings her passion for patient care along with her extensive knowledge of the first responder community, their lifestyles, risk and challenges to our practice. Kepra provides the education, outreach and recruitment for all of our programs.

    Confidentiality

    Confidential Information, such as test results, medical reports or records generated by HeartFit For Duty, which are personal to Patient and which are used in Patient’s further health evaluations and treatment, cannot be shared with the agency.

HeartFit Program Information

What to expect:

During our initial visit we will be drawing blood, doing measurements, and a provider visit. We will also be doing a 1 hour glucose challenge as well as education, so plan on staying for at least 2 hours.

Use the rear entrance, go down Maxfield St. turn into lot near the credit union.

We will be drawing FASTING labs, so please nothing but water after midnight.

Insurance:

Our program will utilize personal insurance. Depending on the type of coverage, out of pocket costs will be minimal. We can give an estimate of cost once insurance information has been evaluated.

In the upper right hand corner, under patient resources, you will find the GET TESTED link as well as the PATIENT PORTAL. Please complete all of the information in the GET TESTED section, include in the comments the agency in which you work. Also, complete all of the health history information in the patient portal.

To be included in the testing, you must complete this information or contact our office. Spouses are welcome to complete the testing and education as well.
